How Safety Incidents Work (Lifecycle)
Understanding the workflow helps tie everything together:
Open - Created when an employee reports an injury during daily sign-off
Reported - A supervisor or admin files an official incident report
Reviewed - A manager/admin reviews and finalizes report details
Completed (OSHA only) - Outcome is added for OSHA recordable incidents
Set Safety Incident Permissions
Before using Safety Incidents, permissions must be configured.
Access Position Settings
Go to Settings
Click Positions
Select the position you want to update
Enable Safety Permissions
Under the Safety section:
Create Incident Reports
Allows users to file reports for themselves and employees they track
Manage Incident Reports
Allows users to view, edit, delete, review, and complete all reports
Note:
Admins automatically have full permissions
Only users with Manage Incident Reports will see Safety in the left-hand navigation
Enable Daily Injury Sign-Off (Optional but Recommended)
This feature allows employees to report injuries when clocking out.
Go to Settings
Click Daily Sign In/Out
Enable Safety Sign-Off
This setting is what creates the Open Incidents workflow.
Navigate the Incidents Dashboard
Go to:
Safety → Incidents
You may see up to three tabs:
Open (only visible if daily sign-off is enabled)
Reported
OSHA Form 300
Review Open Incidents
The Open tab displays injuries reported during employee clock-out.
Evaluate an Incident
Review the employee’s description
Determine whether a formal report is needed
Mark “Report Not Required”
If no report is needed:
Click the three dots
Select Report Not Required
These incidents are not deleted — they remain recorded as processed in daily sign-off records.
File an Incident Report
Reports can be created in two ways:
From the Open tab
Manually from the Reported tab using Report Injury
Steps to File a Report
Click File Report
Select:
Injury or Illness
Whether it is an OSHA Recordable Incident
What is an OSHA Recordable Incident?
An incident is typically recordable if it involves:
Days away from work
Restricted work or job transfer
Medical treatment beyond first aid
Loss of consciousness
A diagnosed injury or illness by a medical professional
Required Fields
The following must be completed before submission:
Injury date
Employee start time
Activity prior to incident
Description of what happened
Body part affected
Incident location
Doctor/Hospital information
3. Click Submit
Where Reports Appear
Non-recordable incidents: Reported tab only
OSHA recordable incidents:
Reported tab
OSHA Form 300 tab
Review an Incident Report
Only users with Manage Incident Reports permission can review.
Steps to Review
Go to the Reported tab
Click Review
During Review
Click Add to fill missing information
Click Edit to update details
Save progress and return later if needed
You do not need all information upfront — reports can be completed over time.
Submit Review
Click Submit Review when all information is accurate.
Complete OSHA Recordable Incidents
If marked as OSHA recordable:
Go to OSHA Form 300 tab
Open the incident
Enter the Outcome
This step is required to fully complete OSHA incidents and may happen days or weeks later.
Export OSHA Reports
OSHA 300A (Summary Report)
Click Export
Select:
Year
Format
PDF: Print and post (for smaller companies)
CSV: Upload to OSHA ITA system
OSHA 300 & 301 Reports
Click Export 300/301
Select year
Download CSV
